Anterior Capsulorhexis Contracture
- Anterior capsule fibrosis is thought to be the resultant of lens epithelium proliferation.
- It has also been reported more commonly with silicone foldable lens implants.
- May complicate:
- Continuous curvilinear capsulorhexis in an uncomplicated phacoemulsification
- Eye with disrupted zonules following trauma
- Small diameter capsulorhexis or eye with pseudoexfoliation
- Complications: reduce vision, zonular stretching, IOL compression, IOL dislocation, ciliary body traction or hypotonous choroidal effusions.
- Preventive measures:
- Complete anterior capsule vacuuming at the end of cortical aspiration
- Bigger diameter capsulorhexis
- Use of single piece PMMA lens with a relatively broad-expanded haptics structure
- Treatment: Neodynium: YAG laser anterior capsulotomy for significant anterior capsule contracture.
